Hundreds of maternity service providers require improvement, according to the Care Quality Commission
Women said they were left in blood-soaked sheets and their babies suffered life-changing injuries due to poor care
Counties with lower-quality care were more common in the South East of England, while outstanding services were more prevalent in the northern parts of the nation. One woman whose baby was stillborn told the inquiry that there was no specialist maternity loss or trauma service in the area she lives. Health minister Maria Caulfield apologised for the report's findings, saying maternity services "have not been where we want them to be".
The report's authors wrote: "In many of these cases, the trauma was caused by mistakes and failures made before and during labour. Frequently, these errors were covered up by hospitals who frustrated parents' efforts to find answers.
