Study: Clinician-owned practices enhance quality of cardiovascular patient care without increasing staff burnout brighamwomens JAMAHealthForum

). Among practices with complete data compared with those with incomplete data, there was less representation of the cooperatives in Oklahoma and the Northwest . Practices with complete data had similar mean burnout rates as those with incomplete data and similar mean aspirin-prescribing rates . However, practices with complete data had slightly higher mean baseline blood pressure control rates and slightly higher mean smoking cessation counseling rates .

Unadjusted differences in structural and EHR-related characteristics of quality and well-being PD practices vs the other practices with complete burnout, aspirin, blood pressure, and smoking data are shown in. We observed differences in the prevalence of quality and well-being PD practices by cooperatives; for example, PD practices compared with other practices were more likely to be part of the Midwest and North Carolina cooperatives.

Changes in burnout and aspirin, blood pressure, and smoking performance differed significantly for quality and well-being PD practices vs other practices . Mean absolute changes in aspirin metric performance were 10.5 percentage points for PD practices vs 2.3 percentage points for others ; mean changes in blood pressure metric performance were 7.7 percentage points for PD practices vs 1.1 percentage points for others ; and mean changes in smoking metric performance were 11.

In multivariable analyses adjusting for practice location, specialty composition, participation in an ACO or a demonstration project at baseline, and cooperative, clinician-owned practices had greater odds of being able to improve quality without increasing burnout than did those owned by a hospital or health system . This translated to an adjusted estimate of 22.4% of 133 clinician-owned practices improving quality without increasing burnout compared with 12.
