Kaylea Titford died in awful conditions in her home after becoming bedbound during lockdown and being left to die in pain by her parents.
A review into the death of morbidly obese teen who died in squalid conditions has found there were missed opportunities to help her.
When paramedics found her body the teenager was lying in soiled clothing and bedding on a mattress crawling with maggots and flies. Her hair was dirty and matted, and her body was unwashed and her skin was ulcerated. During his trial, horrific details emerged that the night before she was found unresponsive Titford texted his daughter when she was screaming, saying “If you have a bad chest, stop screaming".
Now, a child practice review, which was commissioned by the Mid and West Wales Regional Safeguarding Board into the circumstances leading to Kaylea's death have found that there was no care plan in place and she should have been subject to more robust assessments to monitor her health and living situation.
It was noted that a decision to withdraw services which was made before the timeline of the report was "not informed by a current assessment of developmental need". The report looked at how the multi-agency services including the local authority, education system and health officials assessed Kaylea in the run up to her death.
The report found that without a care plan in place she was not supported with adult services. It added that "a child with significant and chronic disabilities such as spina bifida should not be discharged from services if that limits the transitional support they need from paediatric to adult care." Regarding her weight and the fact that the teenager was considered grossly obese at the time of her death, during the timeline of the report her weight" and its impact on her well-being" were not recorded.
