A milk truck-tanker that triggered a crash on a Phoenix freeway in 2021 that killed four people and injured several others was caused by driver fatigue, U.S. transportation investigators said Tuesday.
"Having a strong safety culture is more than just putting policies on paper, companies must live those policies from leadership to rank-and-file," NTSB Chair Jennifer Homendy said during the meeting."I certainly hope this investigation served as a wake-up call for AMT and for the entire industry." The driver also had an opportunity to get five to six hours of sleep earlier in the day. Instead, he had worked substantially more than 12 hours.
"Just because something is legal doesn't mean it's safe," Homendy said."Driving fatigue is deadly. Period."
