If you wait until the results are back or if they wait to order the testing when they're seen by another colleague that's going to be involved in care, you really lose a large amount of time. You really can't make great decisions without having that information. If it is possible that there can be a complete resection, then I think that patient is a candidate for neoadjuvant therapy. If it's not possible based on the extent of disease at the time of diagnosis, then concurrent chemotherapy and radiation followed by other therapy is the best choice.
