The community nurses were praised for the huge impact they have on a patient’s journey by providing individualistic care offering a more personal experience and optimising quality of life to help meet the patient’s wishes. Patients, families, work colleagues and members of the public were invited to nominate a local hero across one of the sixteen award categories with over 900 nominations and 51 finalists named.
