Failures at a mental health hospital contributed to the death of a Nottinghamshire woman who walked out of a locked ward, an inquest has found. Helen Tarry's body was discovered on a farm track near Calverton on Boxing Day in 2022, the morning after leaving the Priory Hospital Arnold.

Returning their verdict, the jury said a number of failures at the hospital made it possible for her to leave, including inadequate risk management, confusing record keeping, a lack of understanding of policies and insufficient oversight by senior staff. They also said there were communication failures between the hospital and police, adding both parties also missed opportunities to escalate the situation.

Inconsistencies in how often she should be observed were also found in her records. The inquest was also told Ms Tarry had managed to walk through the exit of the Bestwood ward on at least two occasions in the weeks before her death. He said they had met at the Malt Cross in Nottingham around 10 years ago where they had an "instant connection". "Our mutual love felt so beautiful and true," he added. “She had a knack for making people feel safe, warm and comfortable. She would help anyone and wouldn't realise what a great impact she had on others."
