Inquest jury has raised concerns over the healthcare of teenager
Agencies responsible for the care of a mentally ill woman repeatedly failed to properly respond to her comments about suicide, an inquest jury has concluded.

An inquest jury concluded that agencies failed to respond adequately or appropriately on seven separate occasions when Chelsea disclosed details of her intention to end of her life by using a 'new chemical'.The jury highlighted a failure in communication between a health professional and the Becklin Centre, and said there was an 'inappropriate expectation' that Chelsea would contact the centre if she needed support.
By failing to acknowledge that changes in Chelsea's patterns of behaviour were an indication of increasing risk of suicide, the Becklin Centre did not consider alternative, more restrictive treatment, the jury added.
